Piercing Forms

Minors to get piercing:

  • Notarized Consent Form, you can get your paper work notarized through your Bank, Post Office, or a Private Notary.​
  • Parent/ legal guardian that gets form notarized MUST come in with ID and be present the whole time.
  • Minor needs form of ID this could include: Birth Certificate, State photo ID, and or Passport
  • Parent/ legal guardian need to have matching last names or proof of guardianship
